It is important that all of you know what truly happened; not the defamatory fable that Michael Flaherty continues to tell. Above is a picture submitted by a family member of a resident abused while in the Taylor Nursing Care ward. Taylor staff denied any responsibility even though she was under their 24-hour care. In what began as an attempt to find fault in me, actually exposed their own faults and the true Evil that lurks within the Taylor Community administration. As you look through the Lifeline records here, ask yourself: How long is it acceptable for your Lifeline to be disabled? Quoting former Taylor Community Trustee, Chris Volpe, "There should not be a moment when your Lifeline is not working". Yet, he nor any of the Trustees have done anything to upgrade the Lifeline systems or address Taylor Community staff's attempt to kill me. Despite the mountains of irrefutable digital evidence we have compiled, no apologies have been issued to me or my family. Just continued lies and denials that there are any issues with their system and refusal to upgrade their antiquated, unserviceable equipment.
We were all led to believe that our Lifelines were to be fully functional 24/7/365 days a year.
On the morning of April 17th, 2018, I found that not be the case. When I awoke that fateful morning, I was feeling very ill and was unable to stand up out of bed. After repeatedly pressing my Lifeline pendant without a response, I slid from my bed to the floor and began the long crawl to my front door. In what seemed like hours, while going in and out of consciousness, my own sheer will to live gave me enough strength to get me to my front door. I was found laying halfway out in the hallway by my neighbor, who called 911, and I was then hospitalized with pneumonia. It wasn't until March 29, 2019 when I received my 2018 records from Lifeline Corporate that I realized how truly fortunate I was to even be alive. Hank Offinger and Heather Bell‘s intentional negligence in maintaining my Lifeline, likely would have caused my death had I not had the sheer will to get to my front door. My Lifeline records show that my equipment was malfunctioning and/or completely disconnected for the majority of 2018! Hank and Heather ignored the Failed Supervision com-log alerts in the weeks leading up to this incident and for over two months afterwards. The records shown in the gallery below are just a sample of the recklessness exhibited by Hank Offinger and Heather Bell all under the watch of CEO Michael Flaherty. The Trustees and Board of Directors condoned their actions and Flaherty has repeatedly stated his support for the staff’s performance even after these records were exposed. None of their actions bear any semblance to their mission statement; as the records show, they provided zero support for my independence, health, or dignity.
I can’t emphasize it enough, for all residents to get your Lifeline records. Michael Flaherty and Hank will tell you there is nothing to worry about... don’t believe them. If your base unit is a 6700 model, insist on getting it replaced. Under their contract with Lifeline, Taylor Community is required to keep the most updated equipment available to you. Check your Lifeline pendant, on the back will have a replacement date. If it is past that date, insist on getting it replaced regardless of what they tell you. Your life depends on it. For those of you that use a walker, it is most important that you get a wrist fall detection bracelet immediately. People have been choked to death from strangulation by the necklace pendant (See simulation in Gallery below) Regardless of what anyone says, the breakaway isn’t always reliable. It’s your life and your right, as well as Taylor's responsibility to provide you with the latest technology available. The majority of Lifeline Units at Taylor are 3-4 generations old and are not serviceable by Lifeline anymore. Don't let Hank, Heather, or Flaherty lie to you like they tried with my family, my doctor, and my therapists.
Here are some of the Codes that almost became my death sentence, most Woodside residents' Com-log Reports from 2018 will have one or more of these, but you’ll only know if you demand to see them. If you have any issues getting them, call my son Mike at 210-835-7602.

FAILED SUPERVISION: Pendant is not communicating with Base Unit, if pressed no signal will be sent and no help will arrive. You will die.
NO A/C HOME UNIT, LO BATT-UN: Base Unit is disconnected from wall socket and battery running low, will die in 36 hours if not plugged back in. Once battery dies, no signal can be sent and you are without the Lifeline service. As the battery dies so will you.
ISSUE NOT RESOLVED: Hank/Heather are ignoring the warning signals that there is a problem with your equipment and have left it to remain malfunctioning. They want you to have a catastrophic event.

The complaint to the Insurance Commission was based on facts , not mere allegations. We submitted Hard Factual Documentation and Technical Data that fully substantiate our claims. None of which was mentioned in their Fraudulent Report. The "investigators" merely relied on conjecture from their interviews with Taylor Staff and considered them as facts. While every element of our complaint was supported by documented factual evidence which refuted all their claims.

No mention of Susan Denopoulos, Head of Clinical Operations and the main suspect in the crimes who, along with Michael Flaherty orchestrated the multiple events that almost killed me. From unplugging my Lifeline medical alert system, to intentionally overdosing my medications and submitting false information to my Doctor. Her involvement was at the top of the list, yet they let her conveniently “retire” 2 weeks into the investigation ie; Elder Abuse Cover Up operation.
Does it strike anyone as curious why it took 52 days after my Son and I were interviewed, to interview Hank and Heather? And 54 days before they interviewed Flaherty and Soucy? That gave them exactly 2 business days to make a decision and tell me whether I could remain in my apartment or not. The timeline speaks for itself; they had no intention of finding in my favor from the very beginning. Never interviewed or mentioned Susan Denopoulos, my son Gerry, my Doctor, my Physical Therapist, nor any representative at Lifeline. All of whom had important factual & enough damming information to prove Elder Abuse and put these criminals in prison.

As evidenced by the text, Hank and Heather continued to create a false narrative. Fact is, Hank never attempted to intervene when Genesis staff was neglecting and refusing to shower me, along with isolating me in my room attempting to de-condition me. Though he wasn't an employee of Genesis, by contract he was my representative from Taylor, who should have had my best interest at heart. When it came down to my discharge meeting from Genesis, Hank made sure to be in attendance and was visibly disappointed when the therapists both agreed that I should stay living independently and not be subjected to Assisted Living.

As previously stated, any confusion in my medication protocol was solely with Heather in her inability to properly monitor medications. Her selective recall of the facts is egregious, and her attempt defer blame in falsifying my medical records is criminal. Apparently, I put too much trust in hiring someone who I was led to believe to be a medical professional qualified to monitor my medications and my safety.
Hank also had a habit of backpedaling whenever confronted and stood true to that role in his interview. On 12/14/18 he clearly stated in the email to my sons, that Taylor Care was providing the services that I needed to remain living independently in my apartment at 227 Woodside.
There is No Confusion in what he wrote. Yet the Insurance Commission allowed him, and assisted everyone interviewed to twist their story to fit the narrative of the cover up while never mentioning the main actor, Susan Denopoulos.

If this was a formal deposition, Flaherty would be in jail for the lies he told here and the actions by his Staff that he self admittedly supported.
The TRUTH is: My Son Mike requested that no one was to discuss moving me to assisted living unless Mike was present as my POA. Hank disregarded his request and entered my apartment unannounced and attempted to persuade me to go into Assisted Living.
Ed Soucy who? He had nothing to do with the investigation, unless it was for administering the payoff.
Fact: My assertion that they needed me out of my apartment is confirmed by the information Soucy gave about of the waiting list for rental units at Woodside.
Fact: I received the termination notice on April 2, 2019, two days after I gained possession of the records proving Taylor’s Lifeline negligence during the crawl for my life incident that occurred in 2018. All of which is documented in the Timeline provided to the State Insurance Commission. This was pure retaliation by Flaherty in a hasty attempt to mitigate damages. The Insurance Commission failed to acknowledge the facts and failed miserably in doing an honest investigation. There was never a legal eviction done, just threats to shut off my Lifeline and all other services. They forced my departure by threatening my safety.

The only reason to keep this confidential, would be to cover-up the corruption at the NH State Insurance Commission. I was never given an explanation as to why the “investigators“ :
1) Omitted the main actor, Susan Denopoulos. Likely because her emails alone destroy her and Taylor’s credibility.
2) Disregarded the timeline that showed the retaliation by Flaherty in terminating my contract 2 days after I exposed the damning Lifeline records that proved Taylor Community's negligence.
3) Disregarded all hard factual evidence we presented. Taylor’s documents actually contradicted much of their own statements.
4) Refused to reopen the case when questioned about the blatant Omission of Evidence.
The NH State Insurance Commissioner John Elias was contacted and informed of his department’s glaring Errors and Omissions in the investigation, but he too refused to reopen. Either they are inept at conducting a thorough investigation or they are being paid off by Michael Flaherty and the Trustees.
UPDATE: The exposure of his criminal involvement in falsifying an investigation has successfully removed John Elias from his position by Governor Sununu. Unfortunately, that is all the State has done and continues to aid in the cover-up of Elder Abuse.

His henchmen know that they have no choice but to follow his orders if they want to keep their jobs. The safety and well-being of residents is discouraged, prioritizing the reckless endangerment and neglect of senior citizens. If family members try to interject the resident will get evicted.

However, investigation revealed falsified documents and unethical practices designed to influence medical decisions, highlighting serious concerns about integrity and safety.

Heather Bell, a licensed nurse in Taylor Community at Woodside, has been associated with medical negligence, record falsification, and endangering residents’ safety. She has zero integrity and takes no accountability for her reckless actions in supposedly caring for the elderly.
Safety Concerns in Seniors' Care
Located nestled in the hillside of Laconia with the total absence of a security gate or security officers, you might wonder who is protecting these 400 defenseless Senior Citizens? If you were to inquire, Taylor claims that the facilities staff are there watching over the facility. (landscapers and maintenance crew) I find it hard to believe that the 6-10 guys I’ve seen blowing leaves or snow around on a daily basis have any sense of what’s going on. Nor do the guys doing maintenance inside the units, have a clue what’s going on outside.
Of course they say there’s always your Lifeline that you can depend on as your fail-safe in case of emergency 24/7/365. That may be a valid point if and/or when it’s working. Read on......
Had I not found the strength of will to crawl for my life when I fell ill last year, I would not be writing this today. You know the facts by now.....Up until the morning of April 17, 2018, I believed that my Lifeline pendant was my fail safe when I needed it. As it turns out, Hank Offinger and Heather Bell were intentionally ignoring the daily Failed Supervision Comlog warnings, putting me in a situation where I could have died in my room. The records show that they continued to ignore the Comlogs well after I was hospitalized and returned to my Apartment.
After attaining my Lifeline records from 7 years back, the documentation showed a pattern of behavior that my machine was disconnected in the month of January for three years in a row from 2017-2019, and malfunctions went unresolved for over to two full months at a time. Taylor’s negligence is further exemplified when Lifeline Corporate informed them on two other occasions that my antiquated unit needed to be replaced immediately. Hank and Heather refused to comply. So much for caring about my safety.
Whoever is insuring this facility needs to know about the lack of security as well as the lack of maintenance on your Medical Emergency Alert system.
